Fuller Street Elementary Documents & Forms
Have you recently moved in to the Maple Valley School District or would like to attend as a Schools of Choice student? Our Registration Packet can be downloaded from home and filled out prior to meeting with our Registrar!
Items you will need to bring with you to register are:
- Proof of Residency ~ An official document that proves you live in the Maple Valley District, such as a Driver's License or a Utility Bill.
- A Certified Birth Certificate for the student(s) enrolling. (Note: This will have a raised seal, and is not the certificate given at the hospital with the baby's footprints.)
- An Immunization Record of the student(s) enrolling.

Prescription Form needs to be filled out and returned to school if your student needs to take a prescription medication daily or for a short time at school.
Non-Prescription Form- needs to be filled out and returned to school if your student would like to be able to have non-prescription meds given to them occasionally at school, such as Tylenol, Ibuprofen, cough drops, Anti-itch creams, etc. This medication needs to be provided from home in the original packaging.
Authorization for the Possession and use of Asthma Inhalers, epi-pens, or prescribed emergency medication - needs to be filled out and returned to school if your student may need any of the following: epi-pen, Benadryl, inhaler, etc.
Permission to Share - is needed so if a student has a life threatening condition such as asthma or allergies, seizures, etc., we can let the teachers and other staff that work with that student be aware of the condition so that it can be treated promptly and correctly.
